One of the only things my staff and patients miss about stopping the EHR is the online intake form that could be submitted to us via email or built in the program we used.
Does anyone have a system/program to create an online form that you can build and have on the website that could be filled out and submitted to our office via email?

I use Acuity Scheduling for my online scheduling, which allows you to build forms and send custom emails when people schedule. I guess its fine. I’m not sure if I can also send attachments. For the service I think its $34/month
