      After watching the Fast Map presentation tonight and watching the new case video I noted that it talks about an intake form being used. There is the history form online, but no intake form. I have my own intake forms, but this there a specific one that ID is using to associate with Fast Map as of yet?

        I have not created an ID intake form yet. Do you feel that yours is insufficient? How?

          We are currently updating ours again and taking out insurance questions. So at the same time since you have wanted everyone to use the ID forms, I was wondering what your intake forms were like. I know ours have several questions for Fast Map use already, but feel like they need to be more detailed to include things such as more environmental factors, stress factors, anxiety/depression factors. Or are you only assuming these things due to medication lists and other diseases listed? Thanks

            A lot of the questions will be answered during your consultation on the ID history form and may need to be probed by the clinician. Better still to have a few basic questions or a place to circle certain common metabolic diseases on your intake as well. My advice is to look at your current forms, make changes as necessary, and be aware of probing questions if the data isn’t stacking up MSK during the consultation.
