      Does anyone have a stock sheet they use to get information from the patient for an event.  Name, age, email, what’s wrong with you, etc. Thanks in advance

        This is what I have done in the past. It gives you enough information and allows you to keep the consult under 15 minutes. In the header I add office information and footer is for the logo. You point out their limited test, relate it to why they are in pain, and how you can help.
