Integrative Diagnosis provides this service/website to you, subject to these terms of use (the “Terms”). The Terms constitute our legal agreement with you, governing your use of www.integrativediagnosis.com (collectively, the “Site”), including all content, information, and services provided on the Site. By accessing, browsing, or using the Site, you acknowledge that you have read, understood, and agree to be bound by the Terms and you are of legal age to form a binding contract with us. We may update the Terms at any time, with or without notice to you. Each time you access the Site, you agree to be bound by the Terms then in effect. If you do not agree to the Terms, please do not use the Site.
You agree that you have purchased a single user license agreement. Your password, username and use of this website are for your own personal use, are non-transferable and subject to the following conditions: 1) username and password will be used only by the purchaser, 2) copy or distribution of any content is prohibited and will be prosecuted, 3) You agree not to access or attempt to access the services by any means other than the interface provided by Integrative Diagnosis LLC, 4) You may not copy, modify or display any of the material.
Several means of copy protection are being used on this site. Your IP address, time of use, and pages visited may be tracked to determine legal use. Video may be embedded with unique identifier information. If video/content is copied it can be traced to the account it was copied from. The owner of the account is responsible for copyright violations. DO NOT GIVE OUT YOUR PASSWORD.
If Integrative Diagnosis LLC becomes aware of any violation we reserve the right to investigate such violations. If, as a result of such investigation, Integrative Diagnosis believes criminal activity has occurred, we reserve the right to contact and refer the matter to appropriate authorities, including but not limited to: law enforcement, state licensing boards, insurance company review panels, and University/College ethics committees. YOU COULD HAVE YOUR LICENSE SANCTIONED.
Indemnification
Integrative Diagnosis LLC provides information and opinion regarding diagnosis and treatment of musculoskeletal conditions. This information is intended for licensed healthcare professionals to consider when making patient decisions. This is not a replacement for your own judgment. You are responsible for your own actions and must obey all laws and regulations pursuant to your jurisdiction.
Various treatment methods are demonstrated for illustration purposes only. You must be properly trained in person to even attempt treatment procedures. Online membership is intended to prepare providers for the hands-on courses and act as a resource between seminars. Online members must attend a hands-on course within 12 months of joining ID. Failure to do so will result in cancellation of your online Integrative Diagnosis membership.
You agree to indemnify, defend, and hold Integrative Diagnosis LLC and its officers, directors, employees, affiliates, agents, licensors, and suppliers harmless from and against all claims, losses, expenses, damages, and costs, including reasonable attorneys’ fees, resulting from your use of the Site or use of any products and services or techniques sold or demonstrated on the Site.
Trademark and Copyright
The site, service and materials are the property of Integrative Diagnosis LLC and are protected by copyright, trademark, service mark, trade dress and other intellectual property rights. You are not permitted to use this material in any way without the prior written consent of Integrative Diagnosis LLC.
This agreement shall be deemed to have been made pursuant to the laws of Massachusetts and the United States. Conflicts shall be settled under this jurisdiction. I agree that damages for each occurrence of breach of this agreement will be set according to actual damages or $1500, whichever is greater.
All content on IntegrativeDiagnosis.com and related websites is the property of Integrative Diagnosis LLC. Content may not be copied, stored or distributed without prior written permission from Integrative Diagnosis LLC.
“Integrative Diagnosis,” the tagline “the art and science of clinical excellence,” the tagline “practice success with clinical excellence” and the circle with “id” are service/trademarks of Integrative Diagnosis LLC. United States Patent and Trademark Office serial #87216632.
“Instrument Adhesion Release (IAR)” and “Manual Adhesion Release (MAR)” are registered service/trademarks of Integrative Diagnosis LLC United States Patent and Trademark Office serial #5,654,365 and #5,654,363.
The “Integrative Diagnosis Pyramid,” “ID Unification Model,” load vs. capacity images, symptom dysfunction relationship images are also the intellectual property of Integrative Diagnosis LLC and may not be used, copied, distributed or shown publicly.

Termination
You acknowledge and agree that we may, in our sole discretion, terminate your access to the Site if your conduct is found to be unlawful, inconsistent with, or in violation of the letter or spirit of these Terms, or for any other reason. We shall not be liable to you or any third party for termination of your access to the Site. Should you object to any terms and conditions of the Terms or to any subsequent modifications thereto, your only recourse is to immediately discontinue use of the Site.
As of March 10, 2020, Integrative Diagnosis online membership will no longer be accepting new members. If you have left ID you will not be able to sign back up.
Certification
If your Integrative Diagnosis Certification is revoked or expired, the provider may state “Formerly Certified Integrative Diagnosis Provider for [Body Parts] from [Date-to-Date]“. Providers with an expired or revoked certification may not use ID® (Integrative Diagnosis), MAR® (Manual Adhesion Release), IAR® (Instrument Adhesion Release), respective logos, trademarked and related images, video, or any other ID material. Course attendance may be listed on a resume or CV, using course title, date, and hours only.
Providers that don’t maintain the certification requirements are de-certified and will have their online membership cancelled.
Refunds
Small Group registration is non-refundable but is transferable to another provider of similar skill upon approval by Integrative Diagnosis. Registration may be transferred one time to a similar Small Group at the discretion of Integrative Diagnosis. There is no charge we receive greater than 14 days request notice for the transfer. There is a $75 charge if notice is 14 days or less.
All risk associated with your travel arrangements is assumed by you. Any individual who cannot attend related to travel or COVID, or cannot find another provider to take their place, will be issued a 50% refund.

Should you become sick or exposed nearing the time of your scheduled Small Group, you can either transfer your registration to another provider of similar skill, or you will be issued a 50% refund.

If you have been exposed to Covid-19 or are feeling symptomatic, we require you to withdraw from participation in your scheduled Small Group.
No Implied Waiver
Our failure to insist upon or enforce strict performance of any provision of these Terms shall not be construed as a waiver of any provision or right.
Online Privacy Policy
We are committed to protecting your privacy. Your information will be used for our internal business purposes only. We never sell or lease any information we collect. We may collect personally identifiable information when you visit this site. When you submit personally identifiable information for product purchases, such as membership to the site, we will use this information to process your order. Personal information will be shared with PACE and State Boards requiring proof of completion of courses. In addition, we may use this information to contact you or respond to inquiries, keep you informed of new products or services, to improve the Site and to otherwise improve the products and services we provide to you. For security purposes we record your IP address and cookies.
Third-party Websites
Links to third-party websites on the Integrative Diagnosis site are provided solely as a convenience to you. If you use these links, you will leave the Integrative Diagnosis site. We do not review all third-party sites, nor do we control or assume responsibility for the content or privacy policies of any of these sites. Thus, Integrative Diagnosis does not endorse or make any representations about them, or any information, or other products or materials found there, or any results that may be obtained from using them. If you decide to access a third-party site linked to our website, you do so at your own risk.
Registration and Account Information
Certain sections of the Site may require you to register as a user or create an account. By registering or creating an account, you agree to provide accurate and complete information and to inform us of any changes to that information. You will only use your own account information to access the Site and will not impersonate another person in your use of the Site. Each registration is for a single user only, unless otherwise expressly provided on the registration page. You are responsible for maintaining the confidentiality of your account and password and for restricting access to your computer, and you agree to accept responsibility for all activities that occur under your account or password. If you believe there has been unauthorized use, you must notify us immediately.
When you purchase products from us, payments will be processed via Stripe, a payments processor, and your personal information is or may be sent to, handled, and stored by Stripe in accordance with their Terms of Service and Privacy Policy which can be accessed through the company’s website at https://www.stripe.com
Web Forums
By posting any comments, feedback, ideas, testimonials or other materials on the Site, you give us the irrevocable right to reproduce, distribute, publish, display, edit, modify and otherwise use your content for any purpose in any form, subject to any legal limitations on such use and the limitations set forth in our Privacy Policy.
We do not assume any responsibility for your content or any other content not produced by us on the Site. We may at any time and in our sole discretion screen, monitor, block, remove or otherwise exercise control over any of your content or anyone else’s content that we determine violates these Terms, any applicable law, or otherwise may expose us to civil or criminal liability or reflect poorly on us. In addition, we may permanently terminate your access to the Site in the event we determine that you caused any such violation. In no event shall such rights oblige us to exercise control over your content or anyone else’s content.
Opinions expressed on our web forums or in any corresponding comments are the personal opinions of the original author and do not necessarily reflect our opinion.

- WARRANTIES WITH REGARD TO ANY CONTENT OR SOFTWARE THAT HAS BEEN MODIFIED IN ANY WAY BY ANYONE OTHER THAN US, AND WITHOUT OUR EXPRESS APPROVAL.
- BY ACCESSING, BROWSING, OR USING THE SITE, YOU ACKNOWLEDGE THAT PROVISION OF CONTENT AND SOFTWARE ENTAILS THE LIKELIHOOD OF SOME HUMAN AND MACHINE ERRORS, DELAYS, INTERRUPTIONS, AND LOSSES, INCLUDING ANY LOSE OF DATA..IN NO EVENT WILL WE BE LIABLE, IN CONTRACT, TORT, STRICT LIABILITY, OR OTHERWISE, FOR:

Grievances
Grievances are handled on a case by case basis. The concerns of the participant are individually assessed by our staff. The accuracy of claims made, events that occurred and impact on the attendee will all be taken into consideration and based on these facts an appropriate offer to right any grievances in a fair and expeditious manner will be made. Our policy on refunds, transfers, etc. are stated above and/or on course and seminar listings.
